SEATTLE – The King County Medical Examiner has identified a second child who died last month in the care of Seattle Children's hospital.

Avery Q. Plaster, a newborn, died in transit to Children's from Evergreen Hospital.

Children's says a nurse gave her medication without a doctor's authorization, although it is not clear if that had anything to do with the child's death.

Details of the cause of death are pending.

Avery was the victim of one of three medical errors last month involving Children's. Another child, Kaia Zautner, died after being given ten times the intended amount of calcium chloride. An adult who was having emergency respiratory problems was given the proper dosage of medicine, but it was administered wrong. That person recovered.

Our hearts go out to those affected by the tragic deaths. No parent should have to bury a child; and, when it does happen, I pray their pain is measurably eased. Unlike many who post, however, I MUST praise the efforts of doctors, nurses and certified nursing assistants at Children's Hospital. My teen-aged daughter has been at Children's for nearly two months now. Without the care, compassion, respect and dignity they've shown, my daughter would certainly be "gone." Please know that if you need competent, kidness, compassionate care, it is available at Children's. The sad fact that lives are lost is heart-breaking and I, too, would be upset to lose my child. It's important to ask yourself, though, do you HONESTLY believe any responsible medical provider would get up one morning and say, "I'm not going to do my best today...in spite of the fact it could cause harm???" I still believe that love, mercy, and great people exist. I know. I've met them. And, they've been there!

I support and will continue to support Children's Hospital. The care they give to children and families is the some of the best in the world. And, though these events tragic, they should not take away from the extraordinary work this hospital does on a daily basis.

when you increase the patient to nurse ratio, the risk of tragic events like these go up.... but then... we as a society keep supporting the profit over safety mentality... don't we.

First, are we 100% positive that any of these were not intentional? Second, 10 times the amount of sodium chloride, that is a mistake that a first year nursing student would not make. When administering a drug to a child, regulations require that it be double checked by a second nurse. Where is the second nurse who signed off on these drug pushes? Just food for thought.

My heart goes out to the families that experienced the tragic loss of a child while in the care of Children's Hospital or the families that have a child that was unfortunately harmed while receiving care there. It is a terribly sad situation. I do, however, want to speak on behalf of Children's Hospital. In general they give outstanding care. Yes, we are finding out about a couple of exceptions, but again although unfortunate it is not common. There are many people in my life that would not be alive today or would not be at their current level of health if it were not for Children's Hospital. Thank you Children's Hospital for the care that you give to thousands of children that wouldn't make it without the help of such outstanding healthcare workers. I feel privilaged to live in an area where my children will be well taken care of if the situation arises.

@ noskills~ Childrens has their oun ambulances for tranports from airlifts, other hospitals, etc. These are Children's nurses on board. These are sad and tragic events taking place at Childre's, but I have to say I am not surprised with the micromanaging and the pressure put on by the higher ups to be the best, mistakes will be made and unfortunately the consequences are irreversable.

I comptley agree with NJIMKO....it is sad and tragic..but come on it was not intended!!!!!! I take my children to Seattle Childrens and will continue to go there, they are so loving and caring compassionate people it it just amazing how much they identify ONE patient!!!! They are the BEST!!!!!!! Oh and I know my spelling sucks!

Come on people. How myopic you all are to think that the "humans" you are condemning purposefully set out that day to "kill" their patients. You obviously know absolutely nothing about what these nurses do on a daily basis. Thousands of decisions are made and actions taken each and every day by nurses that lead to saving their fragile patient's lives. What happened was a tragedy. It should have never happened. However, nobody committed murder that day. I certainly hope you people never make a right hand turn without looking both ways and kill a child in a crosswalk. Or reach for something in your glove box and swerve into a child on a bike killing them. Or have a couple of drinks before getting into your car and drive across the median and kill a pregnant woman. Hopefully the media won't do to you what it is doing to these nurses.

Wait they are blaming this one on Children's Hospital and then it goes on to say that the child was in route to Children's.
