Public hospital execs make big bucks

RENTON, Wash. -- Top salaries at a publicly-funded hospital in Renton, Valley Medical Center, have become intensely controversial since two hospital district commissioners were voted into office. The elected officials call themselves reformers. Others in the community call them troublemakers.

The chief executive officer of the hospital, Rich Roodman, is the highest paid public employee in the state of Washington. Last year he made a base salary of $615,000. He also collected a bonus of $201,201 for meeting performance goals. On top of that he was paid $263,335 in a retention payment.

The five-member board of commissioners who set Roodman’s salary authorizes this annual payment as a way to motivate Roodman to stay on at the hospital. He’s been at the helm since 1983. Most companies provide a retention payment as a lump sum once the executive has fulfilled his or her contractual obligation; not on a yearly basis.

In total, Roodman earned $1,134,837 in 2010 to run Valley Medical Center, which is part of King County Hospital District No. 1. The district collects property taxes from 400,000 residents in Renton, Covington, Tukwila, as well as parts of Bellevue, Newcastle, SeaTac, Black Diamond, Maple Valley and some unincorporated areas of King County.

Roodman makes about 40 percent more than the chief executive officer of University of Washington Medicine and more than double what the executive director of the University of Washington Medical Center earns.

"Do we need to be paying this much? And the answer is no," said Anthony Hemstad, one of the reformer commissioners elected in 2008. “When corners are being cut every which way, public health tax dollars need to be going into maximizing the public health benefit, not the benefit of CEOs.”

Vascular Neurologist Dr. Aaron Heide is the other reformer commissioner. His six year term began last year.

"There are no justifications for making this salary in this current atmosphere," said Heide.

Heide and Hemstad say Valley Medical Center is run more like a private club than a public agency. They ran on platforms to make the hospital’s business more transparent and accountable to taxpayers. But the men say the other three commissioners have no interest in reforming anything.

"I've worked at many layers of government and I've never seen an institution run this way and it raises all sorts of warning bells for me," said Hemstad. "This is an old culture that doesn't want to change," he added.

An example of what the two commissioners say is problematic at Valley Medical Center is the difficulty they have in obtaining information. When they asked for detailed executive pay data, it took four months to obtain it. The numbers weren’t turned over until Hemstad submitted a formal request for public records.

“They certainly didn’t want this [executive pay] to be public, even to the commissioners. If they did they would have given it to us when we asked for it,” said Hemstad.

The KING 5 Investigators had no trouble obtaining salary data from the hospital after submitting a public records request. The reporters found it's not just the CEO, but all top managers at Valley Medical Center who pack home healthy paychecks.

Paul Hayes, the executive vice president, made $588,249 last year, which included a bonus of $154,275 for meeting performance goals.

The senior vice president of medical affairs, Kathryn Beattie, made $489,479. Those figures outpace the top boss at renowned Harborview Medical Center, which is also funded by tax dollars.

The in-house attorney for Valley Medical Center, David Smith, pulled in $352,196 in 2010, which makes him the highest paid public lawyer in the state. Smith makes about two-and-a-half times what Attorney General Rob McKenna is paid.

“Maybe in good times, absolutely. In bad times? I have a tough time when we’re talking about cutting staff and cutting services, and they’re still making more and more and more. I have a problem with that,” said Heide.

KING 5 attempted to conduct on-camera interviews with the other three commissioners and with CEO Roodman. All of them declined.  Board president Sue Bowman did speak with KING 5 by telephone. She said the compensation levels are important to stay competitive. They don’t want to lose top talent to other hospitals.

“I don’t know why Rich’s [CEO] pay is an issue? Commissioner Hemstad brings it up over and over again. I told him, 'Anthony, it is what it is,'” said Bowman. “I don’t think the five-member board needs to keep focusing on compensation. What are we doing for the community? That’s what’s important.”

Bowman also said the board carefully considers research presented to them by outside consultants and attorneys before voting on CEO compensation. Milliman, a healthcare compensation consulting firm, provides the hospital with a full analysis of market comparative data every other year. They consistently find Valley Medical Center’s pay structure is right on target.

John Hankerson, principal and strategic rewards practice leader of Milliman, wrote a memo about his findings to Roodman and Bowman dated February 9, 2011.

“We have consistently found that base pay and total cash compensation have been well aligned with [hospital goals] and that the magnitude of the incentive plan is consistent with other healthcare organizations that are striving to improve performance and quality patient care,” wrote Hankerson.

“We defined the appropriate market [comparable salaries] as ‘where VMC [Valley Medical Center] might recruit executive talent from or where it might lose executive talent to.’ In that light we have included such local organizations as Evergreen, Overlake, Virginia Mason to name just a few,” wrote Hankerson. “In our opinion, the current levels of incentives used at VMC are appropriate and consistent with best practice as well as smart management.”

Senator Cheryl Pflug, the ranking minority on the Senate Health & Long-Term Care Committee, doesn’t think public hospitals should be basing salaries on what non-profit and for-profit institutions pay.

“They pick and choose who they compare themselves to. A much more appropriate comparison would be the University of Washington,” said Pflug.
 
Pflug unsuccessfully sponsored a bill this session that would have banned public hospitals from coming up with salaries by comparing themselves to institutions that aren't taxpayer-funded, such as Swedish Medical Center and Virginia Mason.

“Seriously, is this the Mayo Clinic? No. This is not the University of Washington Medical Center either, which doesn’t pay these kinds of salaries,” said Pflug.

Controversy over money isn't new to Valley Medical Center. Four years ago the Washington State Public Disclosure Commission (PDC) fined CEO Roodman $120,000 after they found the hospital illegally spent tax dollars on mailings, postage and consultants to sway voter opinion on ballot measures in 2005 and 2006.

The PDC called it the biggest case ever involving a public agency misusing taxpayer dollars for a campaign. Valley Medical Center called it a misunderstanding.

In 2009 the Washington State Auditor’s Office found Roodman collected a troubling $1.7 million retirement payment that year, on top of the $900,000 salary he earned in 2009. The auditor found the commissioners authorized this payment “without explanation or public benefit." The auditor also recommended Valley Medical Center should “avoid including similar provisions in future contracts.”

The reformer commissioners say voters put them in place to enact change but it's impossible because they are consistently voted down, three to two.

"To be a little crude, if I were to ask to scratch my butt, it would be voted against three to two," said Heide.

Video of board meetings from the hospital’s website shows dysfunction and conflict within the five-member panel. During a meeting last year commissioners are seen bickering, talking over each other and raising their voices when Dr. Heide continues to try ask questions about executive compensation. He’s told by fellow commissioners that the item is not on the agenda and that he is out of turn and out of order.

"If there is an idea, there is a question, let it be asked, let it be discussed. And that has not been allowed from day one that I've been on the commission. Never. Just sit down and shut up? Correct," said Heide.

Commission president Bowman tells KING 5 Heide and Hemstad create needless trouble and get in the way of progress for the hospital.

“The dissension within the board is very sad because I don’t think they [Heide and Hemstad] understand healthcare,” said Bowman. “They are dead wrong [on most issues.] The things they propose, any board member would vote against.”

I have made a note of who the reformers are (Heide andHemstad) and they are the good guys, as far as I am concerned. I will absolutely not vote for any of the others and I hope all the other people who vote will vote them OUT!!!!

VMC's published income is about $400 million per year. They receive a little over $20 million per year from local property taxes - or about 5% of their actual income. That may not be a huge %, but it is still 5% more of their income than any other hospitals receive so they are indeed in good shape to build out their facilities. The point is that this public facility is paying out much more to their administrators than other public institutions - like Harborview do. Money should be going to health care, not overpaying the admin.

Mr. Roodman and his administration have turned this formerly limited community hospital into an outstanding medical center which provides an extensive array of services to South King County (not to mention jobs created and dollars spent at local businesses).. Tax dollars are less than 1% of the total budget. Mr. Roodman and his staff should be congratulated for their work.

@VMC Coug -- on ignorance, take a look at VMC's website that give the Commissioners' bios. Most people would look at that and say that Dr. Heide & Mr. Hemstad are the most qualified & have the most approrpriate background for the job. This really isn't a partisan issue by any means nor is it a union issue. You think the unions support overpaying management? Bizarre train of thought there... Would think unions, Rs, Ds, & all in-between would be upset about squandering public money. As VMC makes profits year in year out, as most hospitals do, why should it continue to be getting property tax $$$ while no other hospitals in that part of the state do? Especially when they are being misused to pile on $ to CEO & company this really should be changed.

This is one example of why health care cost are out of control. Really? They gave out performance bonuses for meeting goals. Should that bonus not be refunded to the tax payers? My last trip to Valley was when my wife had food poisoning and was puking her guts out. At the time she had no insurance and tried to charge her 1000 dollars for hooking up an IV and 200 dollars for every shot of ondansetron. I know that generic ondansetron only cost five dollars a shot. They lied and said they used Zofran which is brand named ondansetron. I was standing and watching the nurse inject GENERIC ondansetron. So Valley medical Center Lies and steals your money.

@1beachperson. Interesting that you think the quality is 3rd world. My department has a 256 slice helical CT scanner in the ER. Better scanners do not exist. Our ultrasound machines are all less than 3 years old. Stroke patients are scanned within minutes of arrival at the ER, reducing the damage to the brain. There is a radiologist dedicated to the ER 24 hours a day 365 days a year. Our technologists are first rate. My department has three doctors in Seattle Top Docs. It sounds like you have had a bad experience at VMC. For that I am sorry. What I know is that I sweat bullets about the quality of our work. I don't know anyone at VMC who works any less hard than I do.

@whatsyurbeef......and after that, a prudent businessman thinks "Gee, if I take this out in salary I will be taxed.....but if I put it back into my business and expand my market share, I will be wealther in the long run and provide jobs and services to the community. And this is at a public hospital. Private (for profit) Health networks divert one heck of alot more money to their management, and so do (for profit) insurance companies, that is why our health care system is broken and why we should have what EVERY other industrialized nation on earth has, Medicare for all. Privatization of everything, which is what the cons want, does not work, especially for a human necessity like health care.

Dr. Heide, the star of this story, is the stroke director at Auburn Regional Hospital. In other words, he works for one of Valley's primary competitors. It shouldn't even be legal for him to sit on Valley's board. He is a former employee of Valley and left on very negative terms. Anthony Hemstad is on record where he very clearly argues for the sale or dissolving of Valley Medical Center. He is a right-wing republican with ties to a very powerful anti-union organization (Valley has a sizable union presence) and to MultiCare (another competitor of Valley Medical Center). Anything these two say should be ignored because they do not present the truth. Valley Medical Center is a rare government agency that has produced a profit 19 of the last 20 years. Check the records. It's fine to have an opinion but get the facts first. Ignorance is contagious.

King County Public Hospital District #1 is a special purpose district government. It kinda falls between the cracks as it isn't a county government or a state government. It governs itself via 5 elected commissioners, but the majority of those five unfortunately aren't governing, they are following what they are told to do by the administrator. That is how we end up w. the head of a relatively minor hospital getting a king's ransom for a salary. Being a special purpose district too, no one really pays any attention to what is going on there or even realizes that it is a government. Great that Sussanah Frame stepped up and dug in here. It is those forgotten governments where the real problems end up being.

All of you crying about "Obama care," this problem at Valley has absolutely nothing to do with Obama and everything to do with Rich Roodman. He has been in it for the money grab for years. His goal at Valley isn't excellence in healthcare, it's all about manipulating the system for maximum personal gain. The mantra he pounded into his managers at one time was "Make Rich Look Good." He also loaded the Board of Commissioners with hand-picked personal friends who were in his pocket and willing to be manipulated by him. That's why I take every opportunity to vote his cronies out and bring genuine governance and oversight to that bloated Board. You represent me, Dr. Heide, keep up the excellent work and don't let up on the pressure, no matter how frustrating and how many roadblocks Rich tries to throw in your way. For years Rich Roodman has abused his position for personal gain, and clearly executive corruption at Valley continues to flourish. The man is an absolute disgrace.

It is very apparent that large agencies County, Federal and State do not plice themselves and it takes some one to step up to the plate and try to get things into a proper prospective. If we had the time to re-organize all the various government controlled agencies I am sure we could eliminate a lot of unecessary spending. The public should demand it and expect it on all the departments we finance. In the difficult times we are all having we need to make things on a more equal basis. Congrads to the two who are wanting to get things adjusted to a more realistic pay scale.

@ Consuela: Public district hospitals are partially funded by taxpayers. I live in the district, and pay about $200 year to Valley in taxes. Valley is taxpayer supported because they provide care to community residents that do not have insurance and have to pay for care themselves. If you pay taxes but have to be admitted to Valley for a surgery or other care, they count the money you have paid to Valley in taxes during the time you have owned your home in the district, and Valley reduces that amount off your bill.

My taxes pay for those salaries. How do you pay someone 1.7 million retirement payment ontop of his 900,000 salary and he's still there? What is wrong here? People we need to RECALL those 3 followers!!!! They are cutting back on the services but not the salaries,sad.....

This is no surprise, it's been the executive reality for at least a decade. Hire a high priced executive to come in and cut the budget, which means payroll cuts and outsourcing to minimum wage labor. For this the execs receive a bonus equivalent to 3 or more living wage jobs for a year. The new royalty. This greed is destroying the middle class, assuming it hasn't been destroyed already.

NO! this is not President Obama's fault. It is the fault of greed. The state "allows" this to happen yet are cutting off the most vonerable in our state from medical aid. This is not right. yes, it is apples to Oranges here-It hurts all of us. Let me compete for the job. I bet I could do the exact same job for lesser than 60K Per year. Oh that would be a savings of LOADS of cash that could then be used for BETTER ideas. I bet I could manage that office for way less,because to most of us a 60K a year job is more like it. Ever wonder why it cost $400.00 for a blood test? Does it really take that much to get one? no it doesn't. Hospitals make loads of money always have. Doctors make loads of money but it does not make it right or fair for the rest of us

They are staying competitive with the for profit hospitals- hmmmm. Capitalism, a for profit economic system. Trickle down economics- tax breaks for the wealthy/ Ceos and the extra cash flows down to the bottom and EVERYONE benefits. No regulations. This is nothing new, move along good citizens.

Harborview... is a public hospital... Meanwhile you pay increasing insurance premiums to a corporation who is feeding you ads and laughing at your stupidity. Should an organization be allowed to determine how much it chooses to pay for a particular position? Only if they expect to compete with private organizations -- unless you also plan to control prices in the private sector as well. There are a number of places where money is wasted, and often it is in red tape more than the salary of a single person, but talking about that "evil CEO" is more sensational, so easier to grab headlines. The funny thing is, while people are going to "shocked" at that CEO getting paid this much, they will still go out and spend $80 on a pair of denim cut-off shorts because they say "Bebe" on them.

This is exactly why we need Obama care. Main problem is this is a public, tax payer funded hospital, and their compensation should not be compared to private sector equivalent. That's like paying the public defender the highest paid attorney salary. Ridiculous, profits and health insurance companies tying doctor's hands is the problem.
