LAKEWOOD, Wash. -- Washington State Patrol is investigating a fatal accident involving a box truck and a van on I-5 in Lakewood that backed up traffic for several hours Monday morning.

The accident happened around 3:30 a.m. on northbound I-5 just north of Highway 512. West Pierce Fire & Rescue units said a large moving truck rear ended a mini-van that had stopped in the middle lane of I-5. The impact crumpled the metal of the van, trapping the occupants inside.

Rescuers and paramedics rushed to the scene and cut through the van's metal to get to the passengers. The passenger in the back seat of the van, a 24-year-old Lacey woman, was killed.

Two people in the front seats of the van were injured and taken to a hospital. State troopers say their injuries were non-life threatening. The driver of the moving van was transported to a hospital for neck injuries.

"It appears the van began becoming disabled, slowing down if not coming to a stop on the freeway," said Washington State Trooper Guy Gill. "The driver of the semi truck indicated the van came over from lane one into lane two and he struck the rear of the van at that time."

At the height of the closure, three left lanes of northbound I-5 were blocked. Washington State Patrol re-opened the lanes around 8:30 a.m.

Tragic situation. Hope the survivors will be well. Another reason to keep your vehicle maintained. Not sure about this situation, but many times that a vehicle is broken-down on the shoulder is due to the lack of maintenance. A full overhaul is not necessairly needed, but oil change on time, check the spark plugs, fluids etc....will keep 90% of these people moving.

My husband text me from work as I was leaving for work to be aware of a fatality on I-5. My nephew and I passed the accident and I automatically squeezed my nephew's shoulder and was just strickened with grief with the site. Normally I try not to be a lookie loo, but since I was the passenger I looked plus it was bumper to bumper, slow going. I am so very sorry for the person that passed away, I hope she didn't feel any pain, prayers to her friends and family and I pray the other passengers are fine and will heal mentally and physically from the pain of this morning...

@gladimoved: The truck driver was paying enough attention to know the car moved from one lane into another in front of him. Out of all vehicles to cut off, a big truck should be last on the list. They CANNOT stop on a dime. A tragedy for all involved.

Wonder if the van had its flashers on and the driver of the truck was simply not paying attention to what was going on around him/her. RIP lady and I hope the others recover quickly. Another senseless accident. So sad.

That doesn't look like a semi-truck to me, but what do I know I'm not a reporter so I can't report on it for fear I may be right.
